 Job Summary

At NAM, we are undertaking a program expansion of clinical services for patients and their families suffering from addiction and mental health. This expansion will specifically target grow our Therapeutic Communities of Being Initiative, which includes our crisis-capable housing and day programs, tailored to the South Asian community.

 We are currently looking for a Case Manager to join our growing team. The Case Manager will be responsible for enhancing social functioning and social environment of NAM patients. The successful candidate will work as a team with the staff at NAM and will assist in delivering premium care to patients.

 Our team is heavily mentored by our medical director Dr. Harbir Singh Gill, and NAM is prepared to provide a supportive and nurturing learning environment. Though there will be a steep learning curve, this opportunity purports to rapidly enhance comfort working with a mental health, complex trauma and addiction population.

 Responsibilities

·        Review history and undertake risk assessment to determine the eligibility of patients for the program

·        In collaboration with the medical staff, oversee home detox protocol including medical management

·        Evaluate holistic recovery, psychological, social, biological and spiritual facets

·        Serve as the resource patients need for ongoing care, counselling and provide guidance for any patients with critical conditions

·        Deliver quality care while maintaining all metrics pertaining to productivity

·        Assist clients with resources to support and optimize their social determinants of health and wellness

·        Report and record keeping, charting results, assessments, program plans, reports, progress notes, discharge summaries and other client data.

·        Work collaboratively with NAM staff to deliver holistic recovery services to the clients

·        Other tasks may include case management, intakes, orientation, program planning, coaching, crisis intervention, outpatient follow ups, problem solving in relation to NAM growth related tasks

 Required Qualifications/Skills    

·        Registered Nurse or Nursing background or, Social Worker designation

·        Punjabi language fluency is required

·        Be prepared to cultivate and augment existing counselling skills. This will require significant self-directed learning to rapidly build comfort level working with trauma and addictions.

·        Ability to work within a team

·        Strong communication and interpersonal skills, including a willingness to take feedback

·        Previous work experience in the field of addiction and mental health will be an asset

·        Comfort working with diverse language and cultural communities is an asset
